Eligibility:
|
| |
| » |
18 years of age or older;
|
| » |
Not receiving case management
services from the Department of Rehabilitation, Department of Mental Health, Department
of Developmental Disabilities, or MSSP; |
|
» |
Reside
in Fresno or Madera counties; |
| » |
Able to
be maintained in the community through the use of
Linkages services; and, |
|
» |
Have a
need for case management and be willing to participate
in the program |

Other Information:
Linkages is a state funded program offered locally by
the Fresno-Madera Area Agency on Aging. There is not
income criteria and no mandatory fee for service. The
program provides services to everyone in need, regardless
of income level. Contributions are requested of individuals
on a voluntary basis. The program provides comprehensive
case management that includes a thorough assessment
of needs, a client driven plan of action, coordination
of services, advocacy, ongoing support, and education
for clients and caregivers. It is available when you
or someone you know is experiencing difficulties that
hinder independent functioning.
