Program Description: Provides routine services such as reviewing mail, banking and bill paying, monitoring health insurance claims, arranging transportation and home maintenance. Provide periodic assessment and reporting on quality of care to designated family member. Assemble and coordinate a team for other needs as required. Care plan is individualized and strives to bring together the resources needed for a comprehensive care package.
